A conversation, not another form. Our AI intake asks your questions, follows up on what matters, and brings the patient’s story into the visit before you begin.
Built and used by Ahsan Akram, MD, in his own telehealth practice at Ayah Health.
For telehealth, urgent care, clinics, wellness clinics, and practices that bring them together.

Ayah AI
What brings you in today?
Patient
My throat has been sore for three days.
Ayah AI
Are you having trouble breathing or swallowing liquids?
Patient
No. I also have a runny nose and a little cough.
Ayah AI
Have you had a fever, and what have you tried for relief?
More than answers. Context for the visit.
The conversation helps your team understand what to explore next.
Presenting concern
Sore throat for three days
Associated symptoms
Runny nose and mild cough; denies difficulty breathing or swallowing liquids
Relevant follow-up
Fever and measures tried for relief
Ready for the clinician
Review the responses. Decide what comes next.
Your questions. Relevant follow-ups. A better-informed visit.
You see the patient. Let the technology handle more of the groundwork.
Ayah Health Solutions’ proprietary AI technology powers the workflows behind your practice. Whether you see patients online, in urgent care, in a clinic, or through a wellness program, intake connects to note preparation, care-plan drafts, and follow-up.
Combine workflows for a mixed practice. You stay focused on the person in front of you and review, edit, and approve the clinical work.
One sore throat. A whole visit, connected.
Follow a patient’s words from the first chat to the note in your EMR.
Fictional adult patient and illustrative interface. This is a workflow example, not medical advice.
The visit starts before you walk in.
On the couch with their phone or at a kiosk in your clinic, the patient tells the story in their own words. Intake follows up, one question at a time.

“Sore for three days”
History of present illnessAdult with three days of sore throat, runny nose, and mild cough. Denies fever, difficulty breathing, or difficulty swallowing liquids. Reports partial relief with warm tea.
Patient’s intake responses
BackgroundReports no chronic conditions, daily medications, or known medication allergies. Confirm with the patient.
Still to be completed
Exam, assessment, and planAwaiting clinician evaluation. The intake chat does not supply physical examination findings.
Have the conversation. Let scribe help with the note.
With everyone’s consent, scribe captures the encounter conversation in the exam room or during a video call. You confirm the history, describe your findings, and discuss the plan.
Ayah ScribeIllustrative in-person visit
All participants have agreed to recording.
Patient
It’s worse in the morning. My child had a cold last week. I’m still eating and drinking normally.
Physician
Your temperature is normal. There’s mild redness in the throat, without swelling or exudate. Your lungs are clear.
Physician
This looks like a viral infection, so antibiotics won’t help. Keep up fluids, warm drinks, and rest. We’ll review symptom-relief options that fit your history. Please get reassessed if you’re worsening or not improving, and seek urgent care for trouble breathing or being unable to swallow liquids.
Selected encounter excerpts. These findings come from the physician’s in-person examination. A video visit documents only what can actually be assessed.
A fuller note, with you still in charge.
Intake supplies the starting history. Scribe adds the details and decisions from the visit. You check the combined SOAP note, make corrections, and approve it.
Clinical noteIllustrative physician-reviewed example
Subjective
32-year-old adult with sore throat for three days, worse in the morning, accompanied by rhinorrhea and mild cough. Child had a recent cold. Denies fever, dyspnea, or difficulty swallowing liquids; oral intake is normal. Warm tea provides partial relief. No chronic conditions, daily medications, or known medication allergies reported and confirmed during the visit.
Objective
In-person examination: afebrile, well appearing, and breathing comfortably. Mild pharyngeal erythema without exudate or swelling. Lungs clear to auscultation. Findings observed and documented by the physician.
Assessment
Acute pharyngitis with upper respiratory symptoms, clinically consistent with a viral illness after physician evaluation.
Plan
Discussed the likely viral cause and why antibiotics are not indicated for this presentation.
Reviewed hydration, warm fluids, rest, and symptom-relief options appropriate to the patient’s history.
Advised prompt reassessment for worsening symptoms or lack of improvement, and urgent evaluation for difficulty breathing or inability to swallow liquids.
Patient’s questions were answered and understanding of the plan was confirmed.
Illustrative final note after physician review. Clinical decisions and examination findings are never inferred from intake alone.

Not just SOAP notes.
Choose SOAP, History and Physical, APSO, E/M medical decision-making support, or a custom format in your clinical settings. Shape the headings and instructions around your workflow. Every draft remains yours to review; E/M support does not assign billing codes.
Built from the physician’s side of the screen.
Ahsan Akram, MD, built Ayah Health Solutions after experiencing both the useful and frustrating sides of healthcare technology in practice.
He isn’t designing from the sidelines. He uses the platform in his own telehealth service at ayah.health. Clinical work shapes what gets built and what gets improved.

ER triage support is planned for future development, not part of the current release. It will require clinical validation and site-specific review before use, with triage decisions remaining with qualified clinical staff.
